Agent Appointment Process

Thank you for your interest in being appointed to do business with our company.
Please take a few moments to complete the following paperwork.

Agent Appointment Documents:

Download & Complete the following:

• Agency Appointment Application & Profile
• W-9 Tax form

Include the following:

• Copy of E & O Dec page
• Copies of all licenses for agency and producers
• Copy of company generated farm loss runs (past 3 years)

Send to:
Send ALL above appointment paperwork to Holly Glass at holly@jamesalleninsurance.com
or fax to 1-888-860-6083.

After we receive all of your appointment paperwork you will be contacted by a regional underwriting manager to determine if you meet the appointment requirements.  If they wish to have you appointed, an agency agreement will be emailed to you. You are not approved to submit business to us until we receive the signed agreement back.
